About New Vision Behavioral Health Services
New Vision Behavioral Health Services provides mental and behavioral health services to children, adolescents, adults, and families throughout Youngstown and the surrounding Mahoning Valley.
Our team works together to help clients address behavioral health needs while also overcoming everyday barriers that may affect their stability, wellness, and ability to stay connected to care.
We are currently seeking a dependable, organized, and compassionate Case Manager / Qualified Mental Health Specialist (QMHS) to join our team.
Position Summary
The Case Manager / QMHS works directly with clients to identify needs, connect them with resources, build practical skills, support behavioral health goals, and help them remain engaged in services.
This position includes both office-based and community-based work and requires someone who is comfortable managing a caseload, communicating regularly with clients, coordinating with community resources, completing documentation, and following through on referrals and client needs.
Our Case Managers work closely with clinicians, other case managers, nursing staff, front-office staff, families when appropriate, and community partners to support continuity of care.
Key Responsibilities
- Maintain and manage an assigned client caseload.
- Maintain regular contact with clients based on individual needs and service plans.
- Identify client needs related to behavioral health, housing, transportation, benefits, employment, childcare, healthcare, education, and other community resources.
- Connect clients with appropriate community programs, social services, healthcare providers, benefits, and other resources.
- Complete referrals and follow up to make sure clients successfully connect with services.
- Assist clients with appointments, applications, benefits, resources, and navigating community services.
- Support clients in developing coping skills, daily living skills, communication skills, organization, emotional regulation, problem-solving skills, and other appropriate behavioral health goals.
- Help clients identify realistic goals and make progress toward increased stability and independence.
- Follow up with clients after missed appointments, cancellations, or periods of disengagement.
- Review caseloads regularly and identify clients who may need additional outreach, reassignment, re-engagement, or discharge review.
- Coordinate care with clinicians, case managers, nursing staff, families when authorized, community organizations, and other providers.
- Communicate client concerns, barriers, changing needs, safety concerns, or significant issues to the appropriate supervisor or treatment-team member.
- Provide appropriate support during urgent situations and follow New Vision's established escalation and safety procedures.
- Maintain professional boundaries and use trauma-informed, person-centered communication.
- Protect client confidentiality and follow HIPAA and agency requirements.
- Complete timely and accurate client documentation.
- Document the purpose of services, actions taken, client response, progress, barriers, and next steps.
- Complete required case-management records, service documentation, travel/route documentation, discharge documentation, and other assigned records.
- Maintain an organized client schedule and follow through on outstanding needs.
- Meet reasonable client-contact, caseload, service, productivity, and billable expectations for the position.
- Use cancellations and open time productively for client outreach, resource follow-up, care coordination, documentation, and other case-management activities.
- Participate in supervision, team meetings, case coordination, required training, and professional development.
- Maintain dependable attendance, punctuality, communication, and timekeeping.
Minimum Qualifications
- High school diploma or GED required.
- Must meet applicable Ohio requirements to provide services as a QMHS/qualified behavioral health professional based on education, training, experience, competencies, and assigned responsibilities.
- Relevant experience in behavioral health, mental health, social services, human services, case management, substance use recovery, community services, or a related field.
- Strong communication and interpersonal skills.
- Strong organization and time-management skills.
- Ability to complete clear, accurate, and timely documentation.
- Ability to manage multiple clients, appointments, referrals, and deadlines.
- Ability to work independently while remaining connected to a multidisciplinary team.
- Ability to maintain professional boundaries and client confidentiality.
- Dependable attendance and punctuality.
- Reliable transportation for community-based services and client-related travel as assigned.
Preferred Experience
Experience in one or more of the following areas is preferred:
- Behavioral or mental health services
- Case management
- Community-based behavioral health
- Social services
- Resource navigation
- Medicaid behavioral health services
- Trauma-informed care
- Substance use and recovery services
- Crisis intervention or de-escalation
- Housing and benefits navigation
- Working with children, adolescents, adults, and families
- Electronic health record systems
- Community outreach and care coordination
Candidates with additional behavioral health credentials, relevant degrees, or social-service experience are encouraged to apply.

Scope of Role
The Case Manager / QMHS provides behavioral health support and case-management services within their education, training, demonstrated competencies, assigned responsibilities, and authorized scope.
The position does not independently diagnose mental health conditions or provide psychotherapy unless the employee separately holds the professional license or credential required to perform those services.
